COBRA 

When your employment ends at Creighton University, it is important to note how your benefit eligibility changes.  Health, dental and vision coverage ends at midnight on the last day of the month in which your separation of service occurs.  You have the option to continue medical, dental and vision coverage through the continuation provisions of the Consolidated Omnibus Budget Reconciliation Act (COBRA).  Uninterrupted medical coverage prevents you from being subject to pre-existing condition limitations in a medical plan you may have access to in the future.  Here is additional information about COBRA.

COBRA information is mailed directly to your home, so please be sure to contact us with any address changes.

Life Conversion

You may continue coverage after you leave Creighton under the conversion/portability provision of the plan.  You will not be required to provide medical evidence of insurability.  Application and payments are made directly to United of Omaha Insurance Company.   The conversion and portability forms are located below.  Application for continuation of coverage must be mailed within 31 days of your last day worked at Creighton University.
